Als zuweisende/r Ärztin/Arzt können Sie Ihre Patientinnen und
Patientinnen mittels nachfolgenden Formulars anmelden.
Bitte laden Sie das PDF herunter und speichern Sie es lokal mit
den entsprechenden Angaben. Danach können Sie das Dokument
direkt via e-Mail an die Adresse arztpraxis@spital-savognin.ch
senden. Nach Prüfung der Unterlagen kontaktieren wir Sie oder
die Patientin/den Patienten umgehend. In dringenden Fällen bitten
wir Sie, uns direkt telefonisch zu kontaktieren (Tel. 081 669 14 33).
